Built with working physical therapists

Your patients quit at week three. Show them why they shouldn't.

Chart a physical therapy visit in two minutes. UpChart texts the patient a link to a page that shows them, in plain English, that they're getting better.

The problem

What's costing your clinic every month

Patients don't stop coming because therapy isn't working. They stop because nobody showed them that it is.

  • Patients quit before the plan of care ends
  • No-shows and cancellations cost thousands a month
  • The portal is a wall of text nobody opens twice
  • Progress lives in your notes, not in their head
  • Week three is where most people disappear
The fix

Two minutes for you. A reason to come back for them.

No new EMR. No portal. One short entry after each visit, and the patient gets a text.

Scales and radio buttons, not paragraphs

Chart in two minutes

After the visit, open the patient and tap through pain, function, mobility, strength, and home program. Pick the tips they should get. Save.

  • 0 to 10 scales, one tap each
  • Injury-specific measures by body region
  • Clinician-only notes stay private
Why clinics use it

Fewer no-shows. Fuller plans of care.

Patients see the climb

A rising chart and one plain sentence beat a wall of clinical notes. People stick with what they can see working.

Two minutes, not twenty

Scales and radio buttons. No typing unless you want to. Built with a working PT so it fits the visit, not the other way around.

HIPAA from day one

Texts carry no health information. Patient data lives only in BAA-covered services. Audit logs, MFA, encryption. Not add-ons.

How it works

From visit to text in three steps

  1. Add the patient once

Name, mobile number, consent to text, problem area. Thirty seconds.

  2. File an entry after each visit

Pain, function, mobility, strength, home program. Pick the tips you want them to get. Save.

  3. The patient gets a text

"Your UpChart progress report is ready." The link opens a page that says how they're doing compared to last week.

FAQ

Questions clinics ask first

Is UpChart HIPAA compliant?

UpChart is a business associate to your clinic. We sign a BAA with you, and we hold BAAs with every service that touches patient data. Texts contain no health information, only a link with an expiring token.

Does it replace my EMR?

No. Your EMR is for billing and the legal record. UpChart is the two-minute layer that turns a visit into something the patient can see and understand.

What does the patient actually receive?

A text that says their progress report is ready, with a link. The page shows a headline like "You're doing better than last week," a rising chart, the tips you picked, and their next appointment.

What if a patient has a bad week?

The page tells the truth without discouraging them. The headline and tips change based on rules we set with your clinicians. A plateau is shown as a plateau, with context.

How do patients opt in and out?

Consent is captured at intake and recorded on the patient record. Replying STOP to any text ends the messages immediately.

Who's behind it?

UpChart is built by Live Web Studios in North Jersey with a working physical therapist advising on every field and workflow.
